Your Forms Come to You

There is nothing to download here. When you schedule, we text and email your forms so you can finish them on your phone, tablet, or computer before you come in. Most people take about 10 to 15 minutes, and your answers go straight into your chart.

How do I get my new patient forms?

We send them by text and email once your appointment is scheduled. Open the link on whatever device is easiest, answer the questions, and you are set. If you would rather not fill them out on your own, call 907-561-1167 and we will go through them with you, or plan to arrive 20 minutes early and we will help you at check-in.

Why We Ask Before You Arrive

Finishing your forms early lets our front desk check your insurance ahead of time, so we can tell you more about what is covered before your visit instead of after. It also means your exam starts on time.

How Your Forms Work

When you schedule, we text and email you a link. Open it on your phone, tablet, or computer, whichever is easiest, and your answers go straight into your chart. You will be asked for:

- Your contact and emergency contact information

- Vision and medical insurance details

- Health history, medications, and allergies

- Consent and privacy acknowledgments

If You Need a Hand

Not everyone wants to do this on a screen, and that is fine. Call 907-561-1167 and our front desk will go through the questions with you over the phone, or come in 20 minutes early and we will help you at check-in.

Didn't get your link? Check your texts and email, including your spam folder, then call us and we will resend it.

When to Arrive

- Forms already done: arrive 10 minutes before your appointment

- Forms not done yet, or you want help with them: arrive 20 minutes early

- Bring your insurance cards, a photo ID, your current glasses or contacts, and a list of your medications

What should I bring to my appointment?

Your insurance cards, both vision and medical, a photo ID, your current glasses or contact lenses, and a list of your medications. If your eyes may be dilated, bring sunglasses for the drive home.

Can I fill out forms for my child or a family member?

Yes. The link we send can be completed by a parent or guardian, or by anyone helping a family member get ready for their visit. Bring their insurance cards with you.

Is my information secure?

Yes. The forms open in our secure patient intake system and go directly into your medical record. Please do not send health details or insurance numbers through the contact form on our website, since regular email is not secure.
